Description of TCC Stroke

The TeleClinical Care - Stroke (TCC-Stroke) app is for use by patients enrolled in the TCC-Stroke research study. This study has been funded by NSW Health and is sponsored by the University of NSW. The TCC-Stroke app is designed to support patients who have recently been admitted with a stroke, or a Transient Ischaemic Attack (TIA). It allows for the delivery of educational messages, the monitoring of physiological parameters (such as heart rate/rhythm and blood pressure) by a team of hospital-based clinicians. The app also assists with medication adherence and provides a virtual stroke rehabilitation exercise programme. The objective of the app is to educate and empower patients living with cerebrovascular disease to ensure they keep well and out of hospital.
