Entering height is now introduced as a step in CKD onboarding. In the Event to response configuration, for each of the events, the Role of the Care Team Member can be assigned. A new question is Included in the Check-in screen to track medication adherence for a patient on a daily basis. Now, Patient Tracker & User Feedback data are available in RAD. Now the Nurse can add their availability in the Profile section for the Hour-to-Hour configuration. The Care Team members can choose from a list of pre-defined responses to send to the patients.

Description of Reya Orthopedic Remote Care

Reya Orthopaedics Remote Care Application is a home monitoring solution for post-op, which involves nurses doctors and even physiotherapists. The application helps the care team manage their patients through their recovery remotely. Eligible patients are onboarded by a nurse on the smartphone app. Patients use the Patient App to add their daily check-ins of vitals and symptoms, if any. Nurses are immediately alerted of adverse information logged in, allowing quick followup with patients. Doctors generate digital care plans for the patient remotely. The system enables care continuity and prompt feedback loops between the patient and their care team members. This App has been integrated with Apple Health App to fetch Walking + Running distance data. Key Features: Efficient triaging of patients by intelligently generating the Care Team’s queues Categorised and hierarchical historical data representation within the patient’s file to help Care Team contextualise before attending to patients In-app messaging capabilities between the Care Team and Patient In-app messaging capabilities between the members of the Care Team within the context of each patient In-app escalation of the patient within the Care Team structure Scheduled reviews customised based on the type of surgery, which is then used by the Care Team to collect critical information required for remotely monitoring the patient which also helps in dedicating Care Team’s attention to individual patients at a given cadence.
