Keeps asking for receipts when using card
They keep asking me for receipts a week after using my card at CVS pharmacy. Never had this problem with any of the older “legacy” FSA administrators. The app is prettier but I’ve never had to use any FSA apps because the card just worked. Now I have to use this app and scramble to find old receipts every single time I use the card. I like it!
The Forma app is fairly easy to use and upload receipts. The only drawback is it kicks you out from time to time and you have to reauthenticate everything. Other than that, it’s great!Bryce's Mama

So user friendly, great UI, and easy to use. We can not say enough great things. Pretty user friendly
The only reason I’m not giving it 5 stars is because some of the options I feel are not as inclusive for what your claim / purchase should fall under. However, the app is way user friendly and very easy to submit claims and check status of pending claims.Kenalbenz

When my company first announced this program as a benefit I thought I need to research this company and see exactly what they do. I was very impressed that Forma handles so many companies. I had no idea this benefit was an option or even available. I was impressed with the company who told their employees they had to take a vacation away from the company. That made my day. If only my company would say that. I am so happy my company has researched a need for their employees and has offered such a generous benefit. Myself personally I had a hard time deciding what exactly I wanted or needed. My family has always came first so to have an option to purchase something I wanted or needed was thrilling. Yes I still bought what my family wanted but I did purchase some items for myself. Thank you for handling my company’s benefit. So easy and quick
This app is so easy to use. They are fast at processing a request and never give you a difficult time or hold up a claim. I love that we can get reimbursed for things that we actually need and use, not only narrowed down to one category like and hsa.yanaitat
