Great app at keeping me connected to my health care provider. Easy to use. Sends regular reminder messages. Keeps me very connected to my health care provider and on top of any health issues hopefully preventing problems before they arise.

A great app that does everything it claims. Very easy to use, tracks health data, connects automatically with your doctor and health care team at ChronicCareIQ. A nurse will occasionally call to check on you, and if any reading is out of range the nurse or your doctor office will call. So, there are folks actually checking the data you enter, and checking on you. Works seamlessly.
