DiaDirect is a diabetes remote patient monitoring platform, for endocrinologists and primary care physicians to optimize outcomes in accordance with American Diabetes Association standards of medical care. DiaDirect takes advantage of the bluetooth capable iHealth devices for home measurements of blood pressure, weight and capillary blood glucose, and provides patients direct messaging to their nurse. HIPAA compliance: All data is encrypted with multiple robust security measures including Advanced Encrypted Standard 256-bit (AES-256) decryption key. Medicare compliance: Time tracking, data logs, documentation and export features support the criteria necessary for billing codes under Remote Patient Monitoring and Chronic Care Management services as outlined by Centers for Medicare (CMS). USER PERSONAS: Patients - Users who will be onboarded by healthcare staff and follow their healthcare plan. The app will automatically sync measurements from the connected devices to the clinic portal. Healthcare staff - Users who will be onboarded by the admin and who manages the patients' data. Disclaimer - DiaDirect allows patient users to handle measurements independently, and day-to-day device handling is outside the healthcare staff or admin users’ control.Admin - Users who will be managing the healthcare staff and be able to download audit logs, billable hours etc.
