Ensocare Choice

Ensocare Choice is an application that helps patients review their post-acute care options at the bedside. It provides patients descriptions, images, videos, and ratings/quality data, to help them make an informed decision about their post-acute care. Patients can make provider selections, then rank them in their order preference. The rankings are fed back into case management or Ensocare Transition software in the hospital which will further guide the patient's transition out of the hospital setting.
