Won’t work on IPhone
It’s a great idea, but I find it doesn’t work on IPhone. Does not bring up all the answer possibilities on some question types, but if you select the one unlabelled box it does show, it is often the wrong answer and can’t move on because these questions require a response.

Good app but incredibly slow when making selections.
Good app but crashes and deletes forms
Good app really easy to use. But they updated recently and we keep losing forms that have already been filled by client. Plus bit fiddly now they changed location of date tab… bottom right corner. Clients struggle entering DOB.
Good, looking forward to an update
This app is great for consultations. You can go in an edit any template you've created or even use some templates they provide. Its helpful for people that don't know where to start. I am eagerly waiting for information we store to be more organized. For example, all of the forms Kate Walker filled out are under her name when I look her up. I’ve been using this app for over a year now and its kind of bothersome to have to scroll down and search for a completed form I’d like to view.
