Pure Healthcare is an Ohio based palliative care and coordination facility. Pure Healthcare values the unique circumstance of each person we serve and develops an individual-based plan for areas they define as most meaningful to improve health and quality of life. We will collaborate and coordinate with primary and other health care professionals to provide seamless transitions across healthcare settings and ensure the effectiveness and efficiency of health care services. Our approach to healthcare is comprehensive, caring for the complete spectrum of needs for patients and their caregivers. This application is designed for existing patients who are participating in our remote pain management and reporting program.
