the app is very buggy

I cannot schedule appointments several days after registration. called CSR, not fixed.

My nausea was dismissed and the dr and NP didnt care

I was experiencing nausea and I wanted the doctor and nurse practitioner to care about my symptoms and what I was experiencing with the nausea. The nausea was all the time and didn’t seem to go away.

Horrible App

Amateurish, not integrated with any health or fitness aps. Very out of the box and hastily put together to boot. Saw better things built out of my college classes

Lack of a 1-800 number: very long onboarding

Compared to Ro, not very proactive in the onboarding process. Call One Medical to transfer primary care files and it would take 15-30 days. On customer support, there is a self-checkout for medical records. When attempting to upload to the Form Health link through numerous back and forth chats over two weeks, discovered that you cannot upload files larger than 10MB and zip files are not allowed. Form Health doesn’t feel like it’s ready for prime time. It’s a shame that my employer has selected Form to be its sole GLP-1 management service. The lack of being able to talk to a real human is disconcerting for healthcare.

No Sync with Apple Health

FORM as a service is fine. My insurance requires it for a prescription, and the humans have been great. My complaint is they send you a scale to make sure your weight is accurate, and the scale syncs directly to FORM, but there’s no way to sync back to Apple Health. I’m okay with them insisting on using their scale but it’s terribly annoying that I can’t easily get my weight data into Apple. This is a straightforward integration missing from the FORM app.

It's a trap

Form health is a trap. Their business model is minimizing GLP-1 medications spendings for your employer. Your health is not a priority at all. They just pretend they are helping by intrusion into your private life. Form Health is holding you hostage just because your employer decided to try saving money, not even because you made this decision. And moreover, I expect Form Health does not even actually saving money for the employer, because Form services do cost money.

Buggy app

On e they get you in, app stops working. No way to schedule appointments. Won’t prescribe auto refills. Would abandon if employer insurance allowed.

The worst app possible. Can’t even upload anything

Crippling app

Hard to schedule, impossible to get email notifications, hard to use the app, and unhelpful responses from the form health team

Form Health is great. So thankful I was referred to them. I am so thankful for my Medical team.. Kaci, Chivas and Lara.
They are so caring, understanding, supportive, and always there for me. I mess up regularly, but they are right there to get me back on track.. again and again.
If you haven’t tried Form Health, you’re missing out.

Recent updates make rescheduling impossible

What ever they did on the most recent update the rescheduling is impossible to do. The screen won’t pop up and stays stuck at the bottom of the screen with no way to bring it up. Makes me have to make several attempts to reschedule because of how long support services takes to respond to reschedule requests.

Basic

The app itself is ok. The camera is faulty in the app so I end up taking the pictures of my food then uploading in the app. Sometimes the camera in the app will fail to upload the image so you have to upload a picture from your camera roll. The one common sense feature I would think the app could do is allow you to go back and add pictures when you didn’t get to put in a food log entry. Since the camera in the app is unreliable, I just take the pictures of my food and log the food entries later, but life happens- you get busy or forget. If I forgot to upload my Monday food log entry then I should be able to add it in..

No response for over two weeks after $200 fee paid

I paid $200 to begin with my care team and after two weeks have not heard anything and has no care team. Like a scam

Awful response times

I paid a month ago and they take forever to respond and process everything. I have to constantly message them for updates and they don’t provide any on their end. Really awful company to work with

Form health is awful

Bad app usability, impossible to find anything, shows notifications when there are none. Oh, and form health is ethically bankrupt and should be out of business but hey, their app is also just bad.

Horrible App / Avoid at All Costs

I am being forced to use this Alp given my employers insurance plan. This app is truly horrible. It provides zero agency on users to make or adjust their appointments. It has horrible ui/ux and overall functionality. Truly awful experience. Avoid if you can.

Shameful and Predatory

Prime example of what’s wrong with the healthcare system. This app that tracks your web usage outside the app and shares your info with third-party vendors because the company isn’t even equipped to handle data storage for things like scheduling and health tracking in-house. Yet here they are being redundant gatekeepers for something your PCP is already qualified to advise you on.

Let’s call this what it is: another predatory business acting as a guardrail for employers at the expense of patient health. Our kids will be watching documentaries about places like this 20 years from now wondering how we tolerated stuff like this.

Unethical

UPDATE: I’m paying 4x what I was paying when I had access to one of the best hospitals in the country.

I am required to use this program and replace doctors I love just to maintain access to medication I’ve used for over 5 years. I’ll adjust my rating if the program is better than the clinicians I already like and trust, but this business model is unethical and at this time it’s interrupting my care and creating a ton of work for me. Without FormHealth I lost 50lbs and my entire medical team is happy with my current weight, but intake is all about STARTING a journey. This is exhausting and predatory.

Terrible

This app is terrible and the company seems to be horribly managed. I think employees switch add this as their forum as a way to no longer cover GLP-1 medications.

Avoid if possible.

Really horrible app with horrible user UI/UX interface. Doesn’t allow users proper control of their own account settings. Changing anything in personal profiles is a burden. Their message feature lacks basic functionality that you think would be implemented for a med health app.
