Needs Key Adjustments
The app has some nice features and functions, but there are some key limitations that need adjusting and would help tremendously.
First, please allow users to separately specify if they would like “Paragraph Format” for HPI vs Assessment/Plan. I like narrative / paragraph history for the HPI but prefer a bulleted/simplified list for the plan. Currently I have to choose and lose out either way (in my opinion). Some more granular fine-tuning of these personal preferences between sections would be greatly appreciated.
Second, please allow for a much higher character limit on Ambient customization for each section of the SOAP. Especially for HPI and Plan sections, where I quickly run out of room to provide key instructions for the note that the AI should be able to handle. The lack of characters forces me to manually change more of the generated note after the fact than I’d like.
Those are my main complaints about the app as it currently stands. There is room for improvement in other areas too, but the issues listed are the ones that affect my efficiency every day. Really hope the developers are able to introduce these changes the next update 🙏🏻

Still a lot of issues
Been using for several years. It gets the job done but not very well. Trying to look at future schedule is always difficult, showing mixed up dates for appts. Wish they could make it so opening patients doesn’t throw things off
System went down a couple of days ago and still do not have the AI function.
When working the AI function was fair, but not great. Some reason tended to leave out pertinent negatives and other details that would be helpful. Some glitches in the software have been loaded, including un commanded ratio of partially completed notes. This would occur when notes for one patient were put on pause where they get x-rays. The meantime would see the next patient. When returning to the prior patient, the previously done work had disappeared. We understand the company is working on this problem.

Does not work
The app is slow, locks up frequently when I am trying to sign off on documents, takes forever to load tasks and often does not load at all; when I am out of the office I cannot trust that the app will work for on call duties. I have updated the app multiple times, and I am on up to date IOS on my mobile device. Too many glitches and very disappointing. Our practice is trying to find solutions with nextgen but this is terrible.

Mostly works
Mostly works, but the schedule frequently skips up and down as it updates itself and moves right as you select a patient so the wrong chart opens. The AI Scribe mostly works but macros are spotty and don’t consistently insert. The AI Scribe also frequently inserts your diagnosis and plan into the PMHx as if it was what the patient walked in telling you. Lastly the AI cannot generate a ROS for you - just have the expectation that you will have to do that manually 99% of the time.

Why have an EMR that crashes all the time? 80+ physician group and we can’t wait to dump this trash
Bad experience
Glitchy, dictation is getting more inaccurate. Will stop recording text periodically. Will slow down mid dictation. Overall bad app.
Unsatisfied
This app is the worse I have ever used. It doesn’t capture important details of the visit, it duplicates, a lot of things, it cannot interpret the patient from the provider. I would not recommend this to anyone.

NextGen is the worst EMR on the face of this planet.

Doesn’t connect for transcription half the time. Constantly shuts off Face ID login.

Not sure why but my Dr uses this website. Setup is clunky majority of functions are not configured correctly on the website. Therefore do not work.

The programmers of this application are horrible. They are so inept that they gave up on the android platform entirely and discontinued support. That’s crazy considering the amount we pay for this. Also it continues to be buggy and constantly has issues on the iOS platform. How is that possible?
Least favorite app I have
Rarely works. Never works first time. Takes forever to load schedules. Can only view documents once and then doesn’t load again. Has not improved in its existence. Stuck with it right now until we switch ehr entirely.

Ok for records. Awkward and slow going between visits for a quick history review. Voice recognition is pretty rough. Overall not a very functional app if you want efficiency.
