Verdict
The clearest difference is review sentiment: RealRelax Smart at 62% against iMassage's 13%. iMassage's advantage is rating (1.6 vs 1.3) and update cadence (every 2 months vs every 4 months). On price, ads, in-app purchases and monetization model there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
iMassage holds the better App Store score, 1.6 against 1.3. Both are backed by a comparable volume of ratings — 20 and 62 respectively. Written reviews complicate that: 62% of RealRelax Smart's written reviews are positive against 13% of iMassage's, so the app with the better star average is not the one people write more warmly about.
RealRelax Smart ships an update every 4 months, iMassage every 2 months. The most recent releases landed on October 24, 2026 and October 23, 2026 respectively.
