Verdict
The clearest difference is rating: PerfectServe Practitioner at 4.6 against UTMC Way's 4.1. PerfectServe Practitioner also leads on update cadence (every 4 weeks vs every 6 months) and ratings volume (1.8K vs 19). UTMC Way's advantage is ads (none vs shown) and monetization model (free vs free with in-app purchases). On price, in-app purchases and iOS requirement there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
PerfectServe Practitioner holds the better App Store score, 4.6 against 4.1. That said, the two scores are not equally well evidenced: PerfectServe Practitioner has 1.8K ratings behind it against 19, so its average is the more settled of the two. Written reviews complicate that: 16% of PerfectServe Practitioner's written reviews are positive against 25% of UTMC Way's, so the app with the better star average is not the one people write more warmly about.
PerfectServe Practitioner ships an update every 4 weeks, UTMC Way every 6 months. The most recent releases landed on October 21, 2026 and October 24, 2026 respectively.
