Verdict
The clearest difference is review sentiment: IgCares at 100% against CVS Specialty's 35%. IgCares also leads on rating (5.0 vs 4.4). CVS Specialty's advantage is ratings volume (1.9K vs 4) and update cadence (monthly vs every 4 months). On price, ads, in-app purchases and device support there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.
IgCares holds the better App Store score, 5.0 against 4.4. That said, the two scores are not equally well evidenced: CVS Specialty has 1.9K ratings behind it against 4, so its average is the more settled of the two. Written reviews point the same way: 35% of CVS Specialty's written reviews are positive against 100% of IgCares's.
CVS Specialty ships an update monthly, IgCares every 4 months. The most recent releases landed on November 1, 2026 and October 7, 2026 respectively.
