Cannot submit maintenance requests

The app crashes regularly and I cannot submit maintenance requests. It is incredibly unintuitive to use.

In my experience this app does not work. I have deleted and reinstalled a number of times. Have deleted the app and reinstalled again this evening and now it won’t progress beyond the login page! Seriously?!
